STANDARD QUESTIONNAIRES

CUSTOMIZED QUESTIONNAIRES

The standard Short Form, used for over  60% of courses at UA, consists of eleven “core” questions plus six demographic questions. It is intended to provide data for use in annual reviews and other performance appraisals. The Long Form includes all the questions on the Short Form (its first page is identical with that of the Short Form) along with 26 questions addressing different facets of teaching. It is intended to provided formative feedback as well as data useful in performance appraisal. The Lab, Discussion and Studio Section forms are for sections attached to lecture courses, typically taught by graduate teaching assistants.
