• Wait until the instructor leaves the room before you distribute the questionnaires
• Read this statement out loud to students:

"Your feedback is an essential part of evaluating the quality of the instruction you are receiving. Your responses are confidential. No results are released until after grades have been filed. Answer each question sincerely and thoughtfully. Please do not discuss your responses with others until all materials have been collected."

• Hand out the questionnaires to the students.
• Count the leftover blank sheets and record that number in the space provided on the monitor card in the red sleeve on the back. Return the blank sheets to the packet.
• Complete your own copy of the questionnaire.
• Collect the completed questionnaires and write the number of used sheets in the space provided on the monitor card.
• Fasten the envelope's clasp (do not moisten the glue on the flap); place the sticker over the clasp on the back to officially seal the packet. Write your initials on the seal.
• Date and sign the monitor card!!!
• Insert the white copy of the monitor card into the red sleeve and return the yellow copy to the instructor before finals to indicate that you have followed these instructions.
• Take the packet directly to an official TCE drop box.
